Jean M. Kozak, nee Ludwicki, age 91, formerly of South Holland, IL, passed away Thursday, October 17, 2019. Beloved wife of the late John J. Kozak. Loving mother of Cindy (Roy) Rick and Wayne (Janet) Kozak. Proud grandmother of Brian (Meryl) Kozak and Kevin (Rachel) Kozak.  Dear sister of the late Arthur (late Mildred) Ludwicki and late Stanley Ludwicki. Loving aunt of several nieces and nephews. Preceded in death by her parents Joseph and Janina Ludwicki.
