Loving son of Paul and Barbara Cook. Dearest brother of Glenn Cook and Scott (Ann) Cook. Kind uncle of nieces and nephews. Dear companion of Carol Johnson. Former husband of Gina Schmidt, nee Dal Santo. Visitation Tuesday, March 22, 2016 from 2:00 - 9:00 p.m. and Wednesday, March 23, 2016 from 9:30 a.m. until the time of the funeral service at 10:30 a.m. at Smits Funeral Home (2121 Pleasant Springs Lane, Dyer, IN) with Rev. Douglas Cater officiating. Interment Assumption Cemetery - Glenwood, IL. In lieu of flowers memorial contributions c/o The Cook Children Education Fund greatly appreciated.
