Baby Maya is survived by her parents Adam and Amy Lewandowski; two sisters, Emma Faith and Olivia Lynn; one brother, Luke Witold. Visitation Sunday, October 28, 2012 from 3:00 PM - 8:00 PM at the Smits Funeral Home - 2121 Pleasant Springs Lane, Dyer, IN. Funeral service Monday, October 29, 2012 at 10:00 AM at Faith Church (100 - 81st Ave. Dyer, IN) with Pastor Ron Citlau officiating. Interment - Holy Cross Cemetery, Calumet City, IL.
